   • 2.2. Mail address (in case statutory seat does not coincide with the mail address, you must specify your statutory seat).
   • 2.3. Copyright holder’s site address in the Internet.
   • 2.4. The business license (if this business is licensed on a regular basis).
   • 2.5. Contact person of a Copyright Holder (name, job title, phone number, email).
3. The information of the complaint sender:
   • 3.1. Name.
   • 3.2. Job position.

   • 3.4. E-mail.
   • 3.5. Copy of the designation of the authority from the Copyright Holder ( not needed in case the person sending the complaint is the Head of the Copyright Holder Company).
   If the complaint is sent not by the Copyright Holder but by its authorized representative – Individual Person, he should submit the copy of the designation of the authority from the Company to the authorized individual (not needed in case the person sending the complaint is the Head of the Copyright Holder Company).
